If you are a new patient, you can complete a medical records release form to have your medical information transferred from your previous doctor to Dickson Medical Associates. Please complete the form soon after you join Dickson Medical Associates so your physician has your complete medical history.

You may download our Medical Records Release Form here, fill it out and fax it to your previous doctor. 

How Do I Get A Copy Of My Medical Records? 

You may request a copy of your medical records at any time at www.medicopy.net/patients then click the ‘Request Records’ button.
