Please arrive at least 20 minutes prior to your first appointment.

Any applicable insurance copays, deductibles, and co-insurances are due at time of service. Payment in full is required at time of service if you are not covered by insurance. Payment depends upon your insurance type and plan and it is the responsibility of the patient to understand the specific benefits applicable to their coverage. Payments are collected at appointment check-in.

For your protection, we require that patients provide photo identification when presenting for appointments.

Please bring the medicines in their actual bottles so your medical care provider can know the exact medications and dosages prescribed to you.

It is very important that we have your past medical records. Please verify that these records are being mailed or faxed to us from your previous physician’s office prior to your appointment. If you have not already done so, please complete the Medical Records Release form and return to us according to the instructions on the form.
