The Quality Care Coordinator is a member of the quality health care team. They are responsible for promoting interdependent collaboration for a patient’s continuum of care with the member, physician, healthcare team and family to improve or maintain the member’s social, emotional, functional, and physical health status. The focus of the Care Coordinator is to promote whole person health support to the member with emphasis on establishment of routine contact with his/her primary care physician and facilitate specialist referrals as appropriate per physicians’ orders. In addition, the Quality Care Coordinator is responsible for assisting with closing clinical gaps in care and ensuring smooth transitions in care.
